Lost/stolen passport replacement

If your passport is lost or stolen, you cannot simply renew it. You must report it immediately using Form DS-64 and then apply for a completely new passport using Form DS-11 (in-person application).

1. Report the Loss/Theft (Form DS-64)

  • Complete Form DS-64 (Statement Regarding a Lost or Stolen U.S. Passport)
  • Submit it as soon as possible:
  • Online Submit electronically at travel.state.gov
  • By mail Send to the address on the form
  • In person Bring to your passport acceptance facility appointment
  • The State Department will cancel your passport within one business day of receiving the report
  • Once canceled, the passport is permanently invalid — even if you find it later

2. Apply for a New Passport (Form DS-11)

  • Complete Form DS-11 (Application for a U.S. Passport) — do NOT sign until at the facility
  • Gather all required documents (see below)
  • Appear in person at a passport acceptance facility
  • Submit DS-64 along with DS-11 if not already submitted separately
