If your passport is lost or stolen, you cannot simply renew it. You must report it immediately using Form DS-64 and then apply for a completely new passport using Form DS-11 (in-person application).

1. Report the Loss/Theft (Form DS-64)
- Complete Form DS-64 (Statement Regarding a Lost or Stolen U.S. Passport)
- Submit it as soon as possible:
- Online Submit electronically at travel.state.gov
- By mail Send to the address on the form
- In person Bring to your passport acceptance facility appointment
- The State Department will cancel your passport within one business day of receiving the report
- Once canceled, the passport is permanently invalid — even if you find it later
2. Apply for a New Passport (Form DS-11)
- Complete Form DS-11 (Application for a U.S. Passport) — do NOT sign until at the facility
- Gather all required documents (see below)
- Appear in person at a passport acceptance facility
- Submit DS-64 along with DS-11 if not already submitted separately
